In place = 1 point · Partly = 0.5 · Not in place = 0 · Unsure = 0. Unsure is also flagged “needs verification” — unknown is not assumed safe. Questions are weighted (1 useful, 2 important, 3 critical). Domain and overall scores are weighted percentages.
Bands: 80+ good foundations (still confirm with evidence); 55–79 some important gaps; below 55 significant gaps. A completed questionnaire is self-reported. Verified findings appear only after evidence review and are labeled separately. The score is private — not a public badge and not indexed for search as a clinic rating.
